NTSB Probes Fatal Amazon Cargo Jet Crash

Federal investigators are detailing initial findings from a fatal crash involving an Amazon (AMZN)-branded cargo jet at Miami International Airport. The aircraft overran the runway and came to rest about 1,300 feet beyond its end after striking multiple vehicles. Officials reported five fatalities and five injuries among people in the vehicles hit by the aircraft.

National Transportation Safety Board chair Jennifer Homendy described the crash site as devastating, saying there was debris scattered across the area. She said the scene showed extensive destruction from the impact and subsequent ground collision with equipment and vehicles.

Investigators have recovered both the flight data recorder and the cockpit voice recorder from the wreckage. These devices have been sent to NTSB headquarters for evaluation, where specialists will analyze flight parameters and cockpit audio to reconstruct the final moments of the flight.

The NTSB said its teams will remain on site for at least a week to collect evidence, document the wreckage, and interview relevant personnel. Officials emphasized that no determination of probable cause will be made while investigators are still at the scene, underscoring that the inquiry is in its early stages.

The investigation will examine the aircraft’s flight history and the crew’s experience and training, looking for any operational or human-factor issues. Technical specialists will also review aircraft systems and engines to identify whether any mechanical problems contributed to the overrun.

Weather conditions and the forecast at the time of landing are part of the review, as investigators assess how environmental factors may have affected the approach and stopping distance. The NTSB will also analyze the engineered materials arrestor system at the end of the runway and evaluate how the emergency response unfolded after the crash.

The cargo jet was operated for Amazon (AMZN) by 21 Air, and investigators plan to examine the contractual relationship between the two companies. That review will consider how responsibilities for operations, safety oversight, and training are structured between the brand customer and the carrier.

By combining evidence from the crash site, aircraft recorders, operational documents, and organizational reviews, the NTSB aims to identify the chain of events that led to the runway overrun and ground impact. The findings are expected to inform future safety recommendations once the full investigation is complete.
